 £26,421 – £28,770 per annum

This role is based in Colchester and the main duties include processing referrals received either direct or through a wide range of agencies, working within hospital discharge and many other stakeholders (this list is not exhaustive) to identify the most appropriate service, club or support to meet the needs of the individuals that offer practical, social and emotional support that will improve their health and wellbeing.

You will build relationships with the people you are helping by listening carefully to what is important to them and what motivates them.  You will work individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes.

The locations for this role are:

  • Discharge Hub
  • Cluster Wards

You will bring:

  • An awareness of the current health and social care climate
  • A passion for helping people to self-care and self-manage their health and wellbeing whilst having our Purpose & Values at the heart of what you do
  • Experience of working with people with complex needs (we would welcome candidates with a mental health working background)
  • An ability to build strong relationships and work collaboratively with community leaders, local authority, and professionals
  • Excellent communication (written, verbal and listening) skills with an ability to write reports and papers
  • Competent and secure use of IT, including Email, Internet, MS Office 365, Database and Cloud technologies
  • An ability to prioritise tasks, workload and manage time effectively
  • Great interpersonal skills to work with diverse communities
  • Strong communication skills
  • Empathy and emotional resilience
  • Ability to work in a team environment
  • Flexibility with working hours and location this role is outreach and therefore you could be based within the hospital and other partner locations.

 This role would suit someone who has the resilience to deal with complex and sometimes challenging situations.

  • A base in Colchester but there needs to be flexibility with working location (Hospital etc)
