£26,421 – £28,770 per annum

This role is based at Broomfield Hospital, Chelmsford working as part of the Social Prescribing team, supporting hospital discharges from Broomfield Hospital.   The main duties include responsibility for referrals, onward referral to other services, provide ongoing support which could include telephone calls, assistance at appointments, maintenance of databases, provide relevant reports and maintain the CRM database, attending meetings, undertaking home visits/meeting citizens in their home after being discharged from hospital/follow up calls (this list is not exhaustive).  You will work with individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes.

  • Experience of working with people with complex needs (we would welcome candidates with a mental health working background)
  • An ability to build strong relationships and work collaboratively with community leaders, local authority, and professionals
  • Excellent communication (written, verbal and listening) skills with an ability to write reports and papers
  • Competent and secure use of IT, including Email, Internet, MS Office 365, Database and Cloud technologies
  • An ability to prioritise tasks, workload and manage time effectively
  • Great interpersonal skills to work with diverse communities
